DVA Exercise Physiology and Physiotherapy Eligibility

If you have been elected to receive treatment under DVA arrangements, and currently hold a gold card or white card, or have written authorisation, you will have access to critical clinical care interventions for managing chronic diseases and recovering from significant illness or injury.

Please Consider: Treatment, irrespective of eligibility, will be determined solely by assessed clinical requirements.

Getting a Referral

To access exercise physiology and physiotherapy services at Pinpoint Physiotherapy and Sports Medicine, eligible individuals must first obtain a referral from a healthcare provider. Referrals can be provided by:

  • Local Medical Officers (LMO)
  • General Practitioners (GP)
  • Medical specialists
  • Another exercise physiologist transferring the entitled person
  • Hospital discharge planners
  • Treating doctors in hospitals

Referrals must be documented on either a DVA Request/Referral Form (Form D904) or the official practitioner's letterhead or referral pad. The following essential information must be included in the referral:

  • Name of the entitled person receiving services
  • DVA file number of the entitled person
  • Referrer's healthcare provider number (or hospital's provider number if discharge referral)
  • Date of referral
  • Condition to be treated

Ensuring that all required information is included in the referral facilitates a smooth and efficient process for accessing exercise physiology services.
